I have applied for name/sername change in the gazzatte in the prescribed formate along with fees and submitted the affidavit via registere post. No. RG036035735IN Dt. 8/1/2021 from surat HO post office, same is delivered your office to 12/1/2021
The money order sent to your office address from udhna post office wide receipt number 032235100102994010 dt. 7/1/20221. Its considerable period of one months passed, but no intimation of surmane change has been receive yet, It is a matter of regrating. I can not received any gazzatte book nor received my original application return at my home if any reason for query. It is there fore requested to kindly look into the matter and do the neadfull immediately.

I sent one time change my sir name, but not any certificate receive. I wait five month, but no reply from my request. I sent 2nd time fill form for my sir name and than i receive certificate after 45 days. Problem is that i send a Parekh Devansh Jayendrabhai.
